Schedule An Exam
Patients can book their dental appointment by phone calling 1 (855) 4 NO FEARS or schedule an appointment online. Choose your preferred appointment date and time and let our dental staff know if you have any unique dental priorities or if you require prompt, emergency care. Our office will communicate with you beforehand to ask some screening questions. Appointments will be managed to allow for social distancing between patients.
Complete the New Patient Form
Prior to coming in for your dental exam, complete our new patient form and submit it online or download, print and complete the new patient paperwork to bring to your dental appointment. Include your health history and indicate if you would prefer sedation during your dental appointment. We’re interested in learning more about your level of anxiety. Our dental team understands going to the dentist can often be met with fear. Let us know if you are usually comfortable with dental visits or if you generally have apprehension about receiving dental work. Patient satisfaction is our primary focus. What to Bring
If you have not already submitted your new patient form online, complete the new patient form and bring it with you to your appointment along with your dental insurance card, legal photo ID and a list of current medications. Arrive at least 15 minutes before your first appointment for check-in and processing of your new patient form and additional paperwork. We participate with most dental insurance companies and PPO plans and offer no-interest financing plans.
For the health and safety of fellow patients and staff, we also request patients wear a protective face mask when entering the waiting room area and when leaving the dental office following an appointment. Each Three Rivers Dental Group office follows infection control recommendations made by the American Dental Association (ADA), the U.S. Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration (OSHA) and we are happy to answer any questions you may have about the steps we take to keep our staff and patients safe in our practice.
What Happens Next?
A Patient Service Representative will greet you upon entering the waiting room to review or get you started on completing your new patient form and other paperwork. We will review your health history and dental needs and discuss sedation options available to ease any anxiety you may have before your exam. Each new patient receives a dental exam of the teeth, mouth and gums, xrays to review the status of your oral health and a light exam cleaning, with the option to receive fluoride treatment at an additional fee. We will then schedule your next regular six-month dental checkup to establish a regular oral health routine. A followup complex cleaning appointment or dental treatment may be required for some patients.
